Two different mechanisms

Both drugs can make a person feel calmer, and they get there by different routes.

Hydroxyzine

Blocks histamine at H1 receptors. In the brain, histamine helps maintain wakefulness, so blocking it reduces alertness. Its calming quality is bound up with that sedation.

Alprazolam

Enhances the effect of GABA, the brain's primary inhibitory neurotransmitter. GABA dampens neuronal firing throughout the central nervous system, and benzodiazepines increase that dampening.

The GABA pathway is also the one implicated in physical dependence and in withdrawal reactions, which is why benzodiazepine labeling addresses those risks so prominently while antihistamine labeling does not.

This is a genuine pharmacological distinction with real consequences. It is also not a complete safety verdict — it describes population-level mechanisms, not what will happen to an individual.

Controlled-substance status and boxed warnings

Alprazolam is placed in a federal schedule under the Controlled Substances Act, based on accepted medical use and potential for abuse and dependence. Scheduling brings additional requirements around prescribing, refills, record keeping and storage. Hydroxyzine is not scheduled.

A boxed warning is the strongest warning the FDA can require, appearing at the top of the prescribing information inside a border. Alprazolam labeling carries boxed warnings covering risks from concomitant use with opioids; risks of abuse, misuse and addiction; and risks of dependence and withdrawal reactions.

Two cautions about reading this comparison.

Do not read "no boxed warning" as "no risk". Hydroxyzine carries sedation, driving impairment, interaction with other depressants, anticholinergic effects, and heart rhythm considerations that regulators have formally reviewed. Absence of a boxed warning is not a clean bill of health.

Do not read "boxed warning" as "should not be used". Boxed warnings describe serious risks that require attention. Medicines carrying them are prescribed appropriately every day where the clinical situation warrants it.

Stopping them is not the same

This is one of the most practically important differences and it deserves to be unmissable.

Benzodiazepine withdrawal can be medically serious. Alprazolam labeling addresses dependence and withdrawal reactions in a boxed warning, and abrupt discontinuation after regular use is dangerous. Stopping requires a prescriber-supervised plan, often involving a gradual taper.

Nobody should stop a benzodiazepine on their own, and nobody should assume that what they read about one anxiety medicine applies to another.

Hydroxyzine is not associated with that GABA-related withdrawal pathway, but stopping any prescribed medicine is still a conversation with the prescriber rather than a unilateral decision.

What they share

Focusing on the differences can obscure the overlap, which matters for daily safety.

  • Both are sedating. Driving and machinery precautions apply to both.
  • Both interact with alcohol. NIH guidance describes alcohol combined with sedating medicines as increasing drowsiness and impairing coordination and judgement.
  • Both add to other depressants. Opioids, sleep aids, muscle relaxants and sedating antihistamines all stack with either.
  • Both require a prescription.
  • Both need a complete medicine list reviewed by a pharmacist.
  • Neither addresses an underlying anxiety disorder on its own. The National Institute of Mental Health describes anxiety disorders as typically addressed with a combination of approaches.

Why we will not tell you which is better

Readers arrive at this comparison wanting a verdict, and we are not going to supply one.

The choice depends on the diagnosis, the symptom pattern, the person's history including any history of substance use, what else they take, their other conditions, their age, what has already been tried, and their own preferences about trade-offs. None of that is available to a website, and a general recommendation would be wrong for a large share of readers in ways that could matter.
