Accumulation happens when repeated doses are given before the previous one has been substantially eliminated, so the total amount in the body rises over successive days until steady state is reached.
Accumulation is more pronounced with long half-lives and with reduced liver or kidney function, and it can explain sedation that grows over the first days of regular use.
